Understanding VA Pension When a Veteran Enters a Nursing Home or Memory Care
When your parent moves into a nursing home, assisted living, or memory care, their VA Pension can change. Aid and Attendance is an add-on payment for veterans who need help with daily tasks like bathing, dressing, or eating.
In assisted living or memory care, Aid and Attendance often stays available and can help cover the monthly cost. The average yearly cost of a private room in a Kentucky nursing home was about $85,775 (Genworth Cost of Care Survey 2023).
Nursing home care works differently. If your parent is in a nursing home and receiving Medicaid, the VA may reduce the pension to a small amount, sometimes near $90 a month.
There is also a planning tension most families never hear about. Moving assets to qualify for VA Pension can trigger Medicaid’s 60-month look-back period, which reviews gifts and transfers made in the past five years. A wrong move on one benefit can hurt the other.

VA Pension is a needs-based benefit for low-income wartime veterans, while VA Disability Compensation is for injuries or illnesses tied to military service. If your parent needs help paying for in-home care, assisted living, or memory care, you are likely looking at VA Pension with Aid and Attendance.
Does my parent’s income disqualify them from VA Pension?
Not automatically. The VA looks at income after subtracting unreimbursed medical costs, and assisted living or memory care bills often bring countable income down to a qualifying level.
How does the 36 month look back rule work for VA Pension?
Since October 2018, the VA reviews asset transfers made in the three years before applying. If money or property was given away to qualify, the VA can apply a penalty period that delays benefits.
Can a surviving spouse of a wartime veteran receive Aid and Attendance?
Yes. A surviving spouse who did not remarry may qualify for Aid and Attendance through what the VA calls Survivors Pension.
Will getting VA Pension affect my parent’s Medicaid eligibility in Kentucky?

Possibly, but the 36-month look-back rule means that transfer needs a close look before you apply. Depending on when it happened and how much was involved, it may trigger a penalty, or it may fall outside the window entirely.
Do I need an accredited VA attorney to file a pension claim, or can a free VSO handle it?
Free Veterans Service Officers do good work filing straightforward claims, but they generally cannot advise on net worth planning, asset restructuring, or appeals when a claim is turned down.
